Fill out a short registration
form
What a juvenile needs to get a library card:
Ø
A parent or
guardian to accompany the juvenile

Parent/Guardian
bring in proof of current address
o
Parent/Guardian
bring in photo ID
o
Parent/Guardian
fill out and sign registration form
Ø
If a parent or
guardian does not accompany the juvenile
o
Juvenile must
bring in library card registration form already filled out and signed by a
parent/guardian
o
Juvenile’s
parent/guardian must already have a library card at the address listed
on juvenile’s registration form
or parent/guardian must be listed in the phone directory
with address listed on the juvenile’s registration form
If you have a library card from any
Arrowhead Library it will already be set up to work at the Chisholm Public
Library
Once you have a library card you can
check out
books, DVDs,
books on tape and CD,
have access to
Arrowhead Regional Library’s downloadable audio books, and request items
throughout the system.
